The dish is a staple of American street food and diner culture, with roots in the Coney Island hot dog tradition but evolving into a regional phenomenon, particularly in the Northeast and Midwest.

Key ingredients include an all-beef hot dog, a hearty meat chili (often beanless), shredded cheddar or American cheese, diced raw onions, and yellow mustard. A great version balances the snap of the dog against the rich, savory chili and the sharpness of cheese and mustard, while the bun must be steamed or griddled to hold up without disintegrating. The defining characteristic is the sheer abundance of toppings that spill over the sides, requiring a plate and fork.
